This should not be submitted to Basketball England. This form should be completed and given to the team secretary who will then licence the participant with Basketball England via the Online Registration System.

This should not be used for any Non-National Players/Coaches who require a Governing Body Endorsement. Please contact the National Leagues Department for the contract which should be used when employing a player using the Governing Body Endorsement system.

Should any NBL team wish to postpone or reschedule a fixture from the published final schedules, then this is the form to use. (Any fixture change must have the full agreement of your opposition).
This form should be completed when a player wishes to transfer from one club/team to another. Once a player is registered to play for an NBL club they cannot register for another NBL club without completing the transfer form and having the transfer authorised by Basketball England.
This form should be completed by players coming into the country to play NBL basketball and their last club was based outside of the UK. This document can be completed electronically and sent back to the NBL department.
This form should be completed by individuals wishing to register to compete in the NBL for the first time and they have never played basketball outside of the UK
This form should be completed by all non-EEA players who wish to compete in NBL division 2, 3, NBDL or Women's NBL.
This form should be completed by any U10 (born after 31/08/2006) wishing to compete in the Jnr. NBL U14 competition.

When attending Basketball England events the following form needs completing. You can fill one in on the day or beat the queue and print your own off.
This form should be completed for any junior player that is playing up to a senior team. Once completed in full, it should be retained by the club. These forms can be requested to be viewed by Basketball England at any time.
