· Violent Conduct 3 (Continuous violent behavior/physical violence)

· Violent Conduct 2 (Pre-mediated physical violence)

· Verbal Abuse 3 (Continuous & Aggressive Abuse)

· Violent Conduct 1 (Physical Retaliation)

· General Improper Conduct

· Verbal Abuse 2 (Excessive Obscene/Foul Language)

· Unsportsmanlike Conduct

· Verbal Abuse 1 (Obscene/Foul Language)

· Violent Conduct 3 (Continuous violent behavior/physical violence)

· Violent Conduct 2 (Pre-mediated violent behavior)

· Verbal Conduct 1 (Including towards own Team Members)

· Verbal Abuse 3 (Continuous & Aggressive Abuse)

· Verbal Abuse 2 (Excessive Obscene/Foul Language)

· General Improper Conduct

· Verbal Abuse 1 (Obscene/Foul Language)

· Gross Misconduct 

· Violent/aggressive behavior (physical/verbal)

· Theft or Fraudulent behavior 

· Racial abuse/comments

· Substance abuse

· Drunken and disorderly conduct

· Threatening words or actions

· Use of aggressive language and/or body language

· Failure to submit a disciplinary report within the                  regulated time

· Failure to submit a disciplinary report on an official report    form

· Allowing players, coaches or team followers to participate    on a match when they do not have valid licence cards or      do not produce a valid email received from the national      leagues department

· Failure to check a score sheet which subsequently has        mistakes affecting the result of the game

· Arriving less than 20 minutes prior to the scheduled top      off time or failing to fulfill a nomination without a valid        reason

· Failure to ensure that disqualified players/coaches are        ejected from the venue

3rd Offence - Final Warning. No officiating appointments for 4 weeks & a £75 fine

2nd Offence -Written Warning. No officiating appointments for 2 weeks & a £50 fine

1st Offence- Verbal Warning. No officiating appointments for 1 week
